For the purposes of this questionnaire, the term “Institution” will be used to mean the overall University/Company. Staff required to complete the questionnaire are to be any investigator involved and/or supported by the Xenome project. Many questions merely require that you tick the appropriate answer. Some questions need be answered only if a certain response has been given in a previous question. An underlined phrase indicates the condition for whether the question should be answered or not.
